Alabama Community Care is a not-for-profit Probationary Regional Care Organization for Region A and Region C which encompass 23 counties in the northern and western portions of our state.
Alabama Community Care was established in 2014 by Huntsville Hospital Health System, Sentara Health Plans, DCH Health System, and Indian Rivers MHC. Our goal is to partner with providers in Region A and Region C to improve the health of Medicaid recipients through a coordinated and comprehensive managed care model.
As we move forward, we want you to know that we have incorporated suggestions from many providers on how to improve access to care, including:
- Eliminating paper referrals
- Eliminating the 14-day visit limit
- Expanding care management coordination
- Pharmacy medication reviews
- Hospital transition of care
- Enhanced enrollee outreach education and services
- Dedicated staff to partner with providers
